PHOTOREJUVENATION

Non Ablative-Lyra Laser & Aura Laser

Lasers have been used in dermatology for a number of years to help improve facial rejuvenation. The mainstay of lasers for wrinkles has been ablative lasers such as the erbium and CO2. These lasers work by vaporizing the outer skin layers and as a result, patients require healing of about 5-7 days. The nonablative lasers aim to rejuvenate without breaking the skin so there usually is no healing time yet collagen is rejuvenating below the surface.

Procedure

The procedure involves shining a laser light onto the skin which penetrates and passes through the outer skin layers, having desired effect on the deeper internal skin layers. The laser stimulates the underlying collagen and causes contraction. The end result is a more youthful appearance with skin contraction and tightening. The treatment with non-ablative lasers may often be more subtle and certainly no guarantee can be given as to the extent of the results.

Advantages

The Lyra non-ablative laser is suitable for all skin types including fair and olive and is useful for wrinkles, deeper sagging and also patients who do not wish to have any down time. It is also effective if there are red veins and spots on the skin.

Treatment

It is advised that patients have regular treatments on a four weekly basis for approximately 6 to 7 months. The non-ablative treatment is best performed when combining the Lyra and Aura lasers. Treatment sessions are approximately 30 minutes and there may be a little discomfort during therapy.

Post Procedure

After the procedure, makeup can be worn and there may be some slight redness. There are no restrictions post treatments and patients may apply makeup immediately and return to work. New collagen formation may take 6-8 weeks to begin so rejuvenation is a process occurring over time.

Risks

Almost all have no reaction or downtime at all. As with all lasers, no guarantees can be given to the end result and there is a theoretical risk of irritation to the outer skin, which could cause crusting, scabbing and in the worst scenario scarring, pigmentation or loss of pigmentation. It is important that sun protection is observed during the entire treatment course. The non-ablative laser is a new area in dermatology, and has attracted enormous interest around the world.

Combined with other treatments

Photorejuvenation is often combined with glycolic acid peels, facial relaxants which are used to relax the frown lines and smile lines and fillers to enhance lip structure and deeper lines around the nose and lip area.

Suitable candidates

It is used for aged and sun damaged skin especially where patients do not want any down-time. It may also be helpful for types of acne scars. New lasers are currently being offered to treat acne in conjunction with medical treatment.

AURA LASER

Blood Vessel Laser

Aura Laser is a form of light that has been altered in order to produce a uniform high intensity single wavelength beam. The laser can be focused to treat fine blood vessels on the face and neck or attached to a scanner for treatment of larger areas or background redness.

Advantages

The blood vessel laser uses a wavelength of light which is maximally absorbed by blood vessels. The laser is therefore very selective in treating blood vessels and minimizes damage to surrounding tissue. Therefore there is much less risk of scarring as compared to techniques used previously to treat blood vessels.

Treatments

Suitable vascular lesions for laser treatment include:

Port wine stain Veins on the face Rosacea Campbell de Morgan spots Venous lakes on lip Superficial angiomas Spider naevi Telangiectasia Red scars and stretch marks Poikiloderma background redness Leg veins

Commonly asked questions

How long does the procedure take & how many treatments are required?

Procedures can take up to 30 minutes or just a few seconds for minor red spots. Multiple treatments may be required at one month intervals. For example facial telangiectasia (dilated capillaries) usually require 2-3 treatments, port wine stains even more.

How does it work?

The green light in the Aura laser converts to heat when it finds red (ie blood running through the veins). This heat inflames and closes down the blood vessels which usually do not re-open allowing fibrosis and reabsorption to occur. If vessels re-open more treatments may be required.

Does the treatment hurt?

It is relatively painless when a topical anaesthetic cream is used prior to treatment. This needs to be applied one hour before the treatment, not rubbed in, and covered with plastic wrap. Usually there is no pain after the procedure.

What are the risks?

Risks are minimal and may include redness, swelling, crusting, spot bruising, scabbing, hyper/hypo pigmentation and very rarely, minor scarring. In most patients there are no side effects. Most people are able to return to work immediately after the procedure, although there may be some temporary redness and swelling present.

How long do the effects last?

The effects are often permanent, however guarantees of permanency cannot be given. Also it does not prevent the development of new vessels over time. What is the cost of the procedure? The average cost is $380 for a treatment of blood vessels on the face and you may be able to claim from Medicare. Sometimes a test spot is needed first which costs $50 out of pocket. Payment is required at time of treatment.

Why doesn’t every surgeon use the Aura Laser?

The Aura Laser is very expensive and the operator needs specialized, supervised training in order to be able to use the laser safely.
