Slim down - Shape Up with Liposculpture

June 24th, 2010

LIPOSCULPTURE is becoming more popular as patients discover they can lose weight and reshape their problem areas in one procedure.

Dr Daniel Lanzer helped convene the first hands-on conference of the tumescent method of liposculpture, which involves filling the fat with large volumes of a dilute fluid before sucking the fat out.

This has made the procedure safer and more effective and it is now one of the most popular cosmetic procedures.

Dr Lanzer performs liposculpture either with the patient either awake or under a light general anesthetic in his modern day surgery centre.

Different types of liposculpture:

Body Liposculpture: Dr Lanzer can reshape and refine most fatty deposits on a female body, giving a more slim appearance.

Common areas for treatment are the two bulges of the hips and outer thigh, the stomach, inner thighs and knees, calves, ankles and the neck.

Megaliposcution: By having many liters of fat removed, patients can get a real kick-start and feel slimmer and lighter.

Patients undergoing megaliposuction often weigh 80kg to 120kg and have tried all different methods of weight loss.

Breast reduction: Dr Lanzer was the first to publish a large study of making women’s breasts smaller and lighter with just liposculpture. He can remove about one to two liters of fat from each breast. Once the weight is gone, the skin contracts and elevates.

Hi Def Liposcuplture: Dr Lanzer was the first in Australia to be trained in the Hi Def method by the group that introduced the technique last year.

Men can obtain a “six pack” appearance almost immediately and women can get that athletic groove on the stomach. The technique involves combining the new Vaser ultrasound device and a new “artistic” approach.

Male boobs: One of the most common procedures performed by Dr Lanzer is the reduction of “man boobs”. Some men are very self conscious and by removing localised fat and possibly thick breast tissue, the chest becomes flat and smooth.

Why Do it?

People have liposuction for shape issues, such as big hips or thighs. Others do it as a way of achieving more general slimming. Pain and discomfort often plays a role, particularly in female breast reduction. Some people get frustrated with all the exercise they do, for limited results.

How difficult is it to book in?

Dr Lanzer offers a free consultation for cosmetic surgery, during which he gives potential patients detailed and personal information. As Dr Lanzer has his own fully dedicated day surgery centre, the liposuction can take place very quickly. Most patients take either a few days, or a week, off work. Generally no strenuous activity is allowed for two week. Country and interstate patients can send in photographs for a free over-the-phone assessment and discussion.

How private can liposuction be?

Dr Lanzer and his staff are aware that the procedure is private and appropriate confidentiality guidelines are kept.

Many, if not most, of Dr Lanzer’s patients keep their procedure absolutely a secret.

FACE MAKEOVER

WRINKLED and sun-damaged skin can look smoother, cleaner and fresher through lasering. As a result, skin that had the appearance of belonging to a 70-year-old can take on the appearance of a 30-year-old’s skin, with longlasting results, Dr Lanzer says.

For an immediate facial skin lift without excision of facial skin, sutures using small thin threads are passed just beneath the skin to pull the outer skin layer upwards and outwards.

Fat bags below the eye can be removed without cutting the skin on the outside. The fat that causes the bulge and the tired-looking shadow is accessible from the inside surface of the eyelid.

The laser makes a fine-line incision and the fat pops out. There are no stitches and no visible skin marks and the result is a more rejuvenated eyelid, Dr Lanzer says.

Extra skin that tends to hang down over the upper eyelid can be removed by laser surgery performed along the upper eyelid drease. The result is a more “open” eye.

For many people, beauty and liposuction go hand-in-hand - completing the appearance of an individual and complementing their looks perfectly. It is a great way to scuplt the body to the desired image. Some individuals consider liposuction to assist with weight loss and to obtain a younger, more agile body - with the looks to go with it!

Many liposuction patients have an ideal perception of their perfect body - this is often obtained via looking through magazines, watching television and idolizing your favourite celebrities or role models. It is important, however, that all potential liposuction patients learn what is and can be beautiful about their own physical shape - as no two people are the same!
Beauty is about being highly familiar with ones own body - and being able to manipulate it in a healthy, positive manner.

Stretch Marks, Wrinkled Skin, Scars, Cellulite Now Treatable

June 21st, 2010

Enclosed is some interesting information from a well known American dermatologist.
For the last decade fads and bogus claims have promised to remove or reduce stretch marks, wrinkled skin, scars, and cellulite without actual proof. These money-making schemes have brought riches to some entrepreneurs and little pleasure to patients who have opted to have a variety of creams, potions, and treatments applied to their faces, bodies, and especially thighs. At last the wait may now be over as effective treatments have been elucidated to improve and in some cases resolve these age-old cosmetic concerns.

Pioneering research from the University of Michigan Department Of Dermatology has revealed some of the answers to questions of how to regenerate skin. While more research needs to be done, their work conclusively demonstrates that old or damaged skin has frayed collagen fibers which do not stimulate fibroblasts (the cells that produce collagen) to do their jobs efficiently.

In fact, just the opposite. The frayed fibers somehow induce the fibroblasts to curl up and produce collagenase which further dissolves the good collagen that exists. The therapeutic pearl of their work demonstrates that fibroblasts can be made to stretch out and again produce normal collagen, even if they are old in age or worn out by injury. The only caveat is that the fibroblasts must still be present in the affected tissue.

First described in Europe and now elucidated at the U of Michigan, injected hyaluronic acid (aka Restylane and Juvaderm) has been repeatedly shown to stretch fibroblasts and thereby induce them to produce new collagen. Therefore, the rejuvenating effects of injected hyaluronic acid fillers are not only due to their volume but also to new collagen production.

Saline was also tested for its stretching abilities at the U of Michigan and it was found to be less effective than hyaluronic acid. Nevertheless, work by Dr Mitchell Goldman and repeated by me at MetropolitanMD, has repeatedly demonstrated scar remodeling when used to stretch scars in which there has been a loss of tissue volume. And for the coup de gras, further work showed that injury to the epidermis, the outer 1 millimeter of skin, sends a message to the dermis, the bulk of the skin, to stimulate fibroblasts to make new collagen. Now we understand a mechanism for the healing effects of ablative fractional laser therapy.

The bottom line is that lasers that produce a fractionated effect on the dermis such as the Dermaxel fractional, and the Airgent when used in conjunction with hyaluronic acid injections can help improve skin texture.

Photorejuvenation

March 3rd, 2010

Photo rejuvenation with any or all of our lasers (aura laser, revlite laser, isolaz, plasma portrait, and fractional laser) works in synergy to produce the best results.

We often also combine the lasers with other cosmetic procedures for rejuvenation such as fillers, muscle relaxant injections, suture lift, and other cosmetic surgery. Liposuculpture can help shape the face around the neck and jowls but rejuvenation tightens the skin. After a suture lift the best and longest results occur if rejuvenation therapy strats to help tighten the collagen whilst  the skin is lifted.

A number of our beauty cosmetic procedures in our Med Day Spa such as microdermabrasion, airgent, and peels often also complement laser rejuvenation.

It works because the laser light passes through the skin until it hits its target which is the blood in the vessels for the aura laser and the water in the deep dermis for the other lasers.

The laser delivers energy into the deep dermis where the fibroblasts (which make collagen) live creating heat.

This heat tricks the fibroblasts into making new collagen.

The process of new collagen formation takes 6-8weeks and then it crosslinks with the elastin over months.

This is why we say the improvement takes a few months to occur and continues to improve for 6-12mths.

Each time creates a difference but multiple cosmetic procedures for laser rejuvenation usually approx 1mth apart are needed.

There is risk of crust or scab with the laser rejuvenation especially if tan or fake tan or sunscreen or makeup on so come in with clean face and avoid sun.

We recommend 4-6 laser rejuvenation cosmetic procedures and over the years as we cont to age some maintenance laser rejuvenation cosmetic procedures as well.

Pigmentation

March 3rd, 2010

Pigmentation is a common issue for our beauty therapists in our new Medi Day Spa.

And the good news is that in most cases either our beauty therapists or our nurse and doctors can really help.

If the pigmentation is mostly freckles and lentigines, then our laser treatments are your best bet. We have laser treatments which target this pigmentation and crusts it and it just falls off after a few days. And you can still cover up with makeup so no real downtime.

Alternatively our beauty therapists can use peels and microdermabrasion cosmetic procedures in our Medi day spa which makes your skin feel wonderful as well.

When your pigmentation is chloasma it often has a hormonal component to it and we would begin with glycolic peels and hydroquinone. We have a 6% hydroquinone concentrated solution which we use as our depigmenting lotion. After a few of these we can also use our laser if it is needed.

If you have pigmentation you want to treat, come and see our beauty therapists in our Medi day spa or alternatively book in to see Dr Lanzer.

Microdermabrasion and peels are available in Malvern, GlenWaverly, Bentleigh, Ballarat, Werribee and Geelong.

Voluma Rejuvenation

March 3rd, 2010

Mid face filler and cheek bone enhancement are the new breakthrough in cosmetic procedures and rejuvenation.

As we age we often lose our mid face malar fat pad and so as our rounded cheek recede our skin hangs.

Of course sun exposure and smoking also contribute to this loosening of the skin as well as genetic factors leading to hollowing of the cheeks , jowls, and nasolabial folds.

Some of us also find the skin falling to the chin causing marionette lines and depressions.

So volumising the face with our thicker fillers especially to the mid face giving us back those rounded cheeks of our youth often accompanied with filler to enhance the cheek bones  rejuvenates the face and gives back that younger look.

We often combine this rejuvenation with filler with other rejuvenation cosmetic procedures such as laser or suture lift or neck liposuction.

Alexandrite Laser For Hair Reduction

March 3rd, 2010

We use an alexandrite laser for hair reduction.

We have always thought that the laser cosmetic procedure for hair reduction was the best option as it more precisely targets the pigment in the hair follicle.

The Alex laser targets pigment in a large structure so the laser mostly targets pigment in the follicle which is like a bulb/root under the surface of the skin from where the hair shaft grows.

The laser light passes through the skin till it finds the pigment in the large structure /follicle and converts to heat.

So if enough laser light finds enough pigment in the follicle to convert to enough heat to destroy the follicle completely, then that follicle is gone and its hair is removed permanently.

If a fragment remains, it may regenerate into a new follicle which is usually smaller and would produce a finer lighter hair.

So most of the hairs are removed and some are miniaturised.

Also there is only pigment in the follicle during anagen or growth phase so only those hairs in the growth phase are treated in this cosmetic procedure.

And those hairs in dormant or resting phase will need to come into growth phase to be treated.

Therefore multiple laser treatments are needed.

Also as pigment is the target, so lighter hairs (grey, white, blond, and very fine hairs) may not have enough pigment in their follicle to convert to enough heat to be treated.

For the same reason if the skin is tanned or otherwise pigmented (even with fake tan or makeup), then the laser light may heat up the skin causing side effects like crusting scabbing or pigment change..

Also it is important not to wax for a few days as if wax or pluck, then the hair is pulled out of follicle and the laser loses its target. So better to cut or shave hair as this removes hair above surface of the skin leaving target hair and pigment for the laser to hit in the follicle below.

If you have dark skin you may need revlite laser as this laser targets water in a large structure not pigment and is a safer laser in darker skin types, but only works on larger thicker hairs.

Aura Laser & Photo Rejuvenation

March 3rd, 2010

Aura laser is one of our classic lasers. We have been using it in one of its forms for over 13years. We think it is still the best laser for treating blood vessels and spider angioma. It is also a great laser for background redness and flushing, red and purple birthmarks like haemangiomas and port wine stains. And we use this laser for removing and lightening pigmented marks like freckles lentigines and chloasma.

The Aura laser is also used for a cosmetic procedure called photo rejuvenation. Photo rejuvenation is where we use the laser light to heat up the deep dermis where the fibroblasts live and then the fibroblast cells start to make new collagen which then crosslinks with elastin and plumps up and smooths the skin hence rejuvenation making the skin look as it was when you were younger.

But the best bit about the Aura laser is not just how effective this laser is, but that the laser has almost no downtime especially for rejuvenation. So it really is a walk in walk out laser treatment or cosmetic procedure in the case of photo rejuvenation.

An ideal person for aura may have some veins and pigmented spots also that get removed.

Facial Fillers

March 3rd, 2010

Fillers are a great cosmetic procedure for rejuvenation, softening lines scars and depressions, enhancing lips, cheek bones and eyebrows and replacing lost volume in the face which often shows up as sagging skin or jowles or nasolabial cheek folds.

Hyaluronic acid filler works like medical grade clear poly filler lifting out and filling lines depressions and scars. Also used for volumising face and often place across lines as a scaffold to reduce ongoing folding of the skin.

Our hyaluronic acid fillers are bioidentical across all species so allergy is extremely rare and we have never seen any so no testing is required.

This hyaluronic acid is identical to the molecule sitting within the 3 dimensional crosslinking of collagen and elastin in our skin and holds water giving our skin its full feel (both soft and firm).

So because it is hydrophilic and holds water it can lead to some initial swelling, which settles in a day or two.

Other risks include bruising from the needle (not the product), irregular or uneven filling which can be fixed, cold sores if you are prone to get them, and even rarer risk of infection or granuloma.

As hyaluronic acid fillers are acidic, they do sting when the filler is injected and usually local anaestheic is required.

It is not permanent and does need to be redone.

How long a filler lasts depends on which filler is used. Other factors such as position (less for lips and longer tear troughs and acne scars),

movement (so lasts less in lips of smokers), metabolism ( athletes and dancers break it up quicker) also determine when the filler needs to be redone.

Muscle Relaxants

March 3rd, 2010

Muscle relaxants aren’t just for wrinkles , we also use these for non cosmetic procedures such as hyperhidrosis (or excessive sweating usually in the underarms palms or soles).

And we also treat some patients for headaches. Although headache is an accepted side effect for a small number of people from the muscle relaxant for frowns, many more of our patients tell us that as the tension goes from their forehead so do their migraines and headaches.

Also we can reduce the bulk of our masseter muscle which we use for chewing or clenching our jaw and can lead to TMJ or jaw pain or teeth grinding. Reducing this muscle with our muscle relaxants for these indication is not a cosmetic procedure but some women with large masseter muscle bulk have a square jaw and we can soften their jaw line to a more feminine straighter line by using the muscle relaxant in this case as a cosmetic procedure.

But most people still want their cosmetic procedure muscle relaxant for reducing wrinkles and browlifts and stopping their frown and crows feet. Also some use it for reducing the DAO muscle which turn down the corner of the mouth and or contributes to marionettes lines.

We can even use our muscle relaxant injections to reduce the neck strap muscles and this also lifts the face.

Again we usually combine this cosmetic procedure with other cosmetic procedures for rejuvenation such as laser, filler, suture lift, neck liposuction, and also some of our beauty treatments in our new Medi Day Spa.

Dr Lanzer often finds that muscle relaxants immediately after an erbium face resurfacing enhances the result and the length of the result.
