HOW DOES ONE TELL IF ONE REQUIRES A BREAST LIFT?

When breasts sag due to weight loss, aging, and post pregnancy, the nipple becomes located below the fold under the breast. This fold is called the inferior mammary fold. When the nipple sits below the fold, it is called ptosis.

There is a form of ptosis called pseudo ptosis where the nipple is still in the correct place but the actual breast tissue sags below the fold. In most cases if there is true ptosis of the nipple, a breast lift may be required. Unfortunately, many people do not want to have a breast lift because of the scarring that occurs with this procedure. During a breast lift, there is a cut all the way around the entire nipple and from the nipple down to the inferior mammary fold and across the fold. Scarring is not ideal on the chest and therefore, this surgical lift is avoided wherever possible.

CAN THE BREAST IMPLANT HELP BREAST DROOPING?

This case shows that sometimes a breast implant can give a breast lift effect. In the pre-photographs one can see that the nipple is 2-3 cm below the inferior mammary fold and there is a sagging, sunken in appearance of the breast. Once the implant has been placed under the breast, it not only expanded the breast but it also gave a definite illusion and lifting effect.

WHAT SIZE DID THIS LADY CHANGE FROM?

This lady changed from a size A-B to a C-D. we never give an exact prediction of the breast size as the way people assess bra sizes is so non-accurate. Bra companies vary how they label bras and also the actual bra cup is often determined by the actual shape of the chest.

AT WHAT PLANE WAS THE BREAST IMPLANT PLACED IN THIS PATIENT?

The implant was placed under the muscle of the chest. This is a deep implant below the breast tissue. The advantage of a deep implant is that it may have less risk of capsular contraction. The implant is always protected by the muscle and it feels more natural being a deeper implant. An implant that is under the breast has less likelihood of interfering with future mammograms.

HOW DOES ONE GET AN IMPLANT UNDER THE MUSCLE?

The procedure was performed through a direct small incision in the inferior mammary fold. As the saline implant was expanded inside the body, not outside the body, a small incision is only required and the deflated implant is put in position and then filled inside the body.

Once the incision is made in the inferior mammary fold, the area is dissected down to the muscle of the chest. Using the surgeon’s finger the muscle is elevated and the implant is placed under the muscle.

WHY SHOULD I CHOOSE A SALINE IMPLANT AS OPPOSED TO A SILICONE IMPLANT?

Dr. Lanzer performs both implants. It is the patient’s preference that ultimately decides which implant to do. There are advantages and disadvantages to both implants. The main advantage of a saline implant is that it does not contain any toxic material of silicone which may one day be an issue. The incision is also smaller with the saline implant.

WHAT ARE THE DISADVANTAGES OF AN ANATOMICAL IMPLANT?

The disadvantage of an anatomical implant is that they have a particular shape. This now means that there is a new 3-dimensional issue. If the implant rotates at all, it may need to be replaced.

Implants do sit high initially. However, after a month sometimes they are nearly in place. Generally we talk about 3 months until most of the implant moves into position.
