Makeover Before & After Photos
The patient above underwent a full face makeover. This includes full face laser resurfacing, upper and lower laser blepharoplasty, face and neck mini lift, facial fillers in the cheeks.
One of the most common issues that I see with patient that attend for a consultation, that they have fairly good knowledge about different procedures but do not have the understanding as to what is the most appropriate procedure for them.
The first issue that helps me decide what is the right procedure is that I ask the patient what bothers them when they look in the mirror. In the case above, the lady commented that she looks tired, angry, and old. The changes that cause that are seen around the eyes, particularly with the heaviness of the upper eyelid and the shadow of the lower lid. The angry appearance is often related to the changes around the mouth with drooping of the skin. The old appearance is often related to the wrinkling of the face particularly around the eyes and mouth and the concave appearance of the cheeks.
Generally speaking, a younger appearance has a more open upper eyelid where one can apply makeup, does not have the shadow of the lower lid which tends to give the appearance of lack of sleep. The skin is smooth and tight and the cheeks have a convex curved appearance as one would see especially in children. The lower face does not have the wrinkled or loose sagging.
The changes that cause the physical features we have described are often related to sun damage, excessive facial movement, nicotine and smoking, and general aging which causes thinning of the skin, loss of elasticity and loss of deep structure, particularly muscle and bone.
With the lady above, we performed the following procedures:
CO2 laser blepharoplasty removed the heavy skin and fat of the upper eyelid. In the post photograph one can see a more natural upper eyelid where she has been able to apply makeup.
This photograph particularly was shown with makeup to show how the makeup can enhance the results of surgery. Makeup which is applied to a more prominent upper eyelid and smooth skin has a compound effect. Makeup applied to wrinkled skin does not usually work. The other advantage of makeup is that it protects her skin from future sun damage.
Lower lid blepharoplasty. Fat collects in the lower eyelid which causes a circular ring shadow below the eyelid. This is how someone would look if they had lack of sleep. The procedure is performed from inside the eyelid. There are no sutures and no visible cutting marks. Once the fat is removed, the skin re-drapes itself more appropriately to be smoother.
This lady had very wrinkled skin, particularly on her cheeks and around her eyes. The laser erbium resurfacing took off many outer layers of the skin. A new layer re-grew from the follicles that are present on the skin of the face. Once the skin had regrown, it was smooth and tight as can be seen in the photograph post-operatively. Dr. Lanzer has used many lasers over the years and has found that the contour erbium laser is the safest in his hands.
Many patients today do not require deep laser resurfacing as in this case and would rather have fractionated lasers such as Fraxel or Dermafraxel.
Mini face and neck lift. The mini face and neck lift involves cutting away all the loose skin of the lower face and neck. Long Term stitches are carefully placed in the outer muscle in front of the ear and below the ear to elevate the drooping skin. Any excess skin is removed. Usually Dr. Lanzer will perform a superficial method liposculpture of the neck at the same time for enhanced skin contraction.
Cheek implant. There are a number of types of cheek implants. Dr. Lanzer offers hyaluronic acid injections which last approximately 18 months. Dr. Lanzer also specialises in stem cell fat transfer which may be a more long term implant. Fat is taken from an area such as the hips or stomach. It is processed in order to concentrate the stem cells. These are then injected into areas of the face such as the cheeks, lips, and eyebrows. Fat can be stored in the bank for future use.
