One of the interesting questions that come up after the cosmetic surgery and often before cosmetic surgery is, “How old do I look?” and “How many years have been removed by me having surgery?” When judging a person’s age, Dr. Lanzer looks at the skin texture for wrinkling, at the eyelids for heaviness and loss of upper eyelid, at the drooping of the skin particularly seen around the mouth and jowl area, and at the angle of the neck.
The before photograph of the lady does have the appearance of a patient in her mid to late 70’s. There is wrinkling all the way down the cheeks and around the eyes. There are also lines on the upper lip. The upper eyelid cannot be seen because of the excess skin which is wrinkled and there is also hooding of the skin so that excess skin can be seen on the outer side of the upper eyelid. There is a small shadow of the upper eyelid which causes an aged effect. There is also significant drooping of the cheeks. Note the heaviness of the lower cheek at the junction of the lower lid and the cheek and also the prominent jowls with sagging on the lower jaw line. Finally, the angle of the neck indicates a sagging and drooping and there is no definition. Each of the above changes is an aging effect and collectively gives the appearance of quite an old lady, certainly beyond her years.
In the post photograph we have intentionally included make-up to point out how makeup can enhance a result as well as give patients better skin protection. However, makeup cannot cover the aged effects that we have described earlier. In the post photograph one can see that nearly all the lines on the cheeks have gone and the skin is smooth. Makeup would not cover the lines. Secondly, one can see her entire upper eyelid which now could have some eye shadow. Note that the sagging and hooding of the outer eyelid has also mostly resolved. The sagging of the mid-cheek to the upper lip has been improved and she now has a more natural, youthful curve rather than the sagging fold. The jowls is probably most dramatically changed in this lady as she now has extremely tight neckline and the sagging and drooping and heaviness of the neck has totally been removed. Interestingly, she now no longer has a shadowing effect of her face but rather a shadow under chin because of the very sharp margin. One can visualise the area in front of the ear and that the scars from the mini face and neck lift surgery are not visible. Makeup certainly can be used to cover those areas.
A question that could be asked of “How long would she expect these results to last?”
I would expect that the eyelid surgery would last her for most of her life as would the changes on her neck and skin texture. The lifting of the face with the mini face lift can relax in time.
