The term liposculpture, I find, is very confusing amongst the public. People often ask me what is liposuction, what is liposuction, and what are the costs of these procedures? Liposuction really means purely sucking out the fat, whilst liposculpture gives the illusion that one is sculpturing away fat in much the same way that an artist would sculpture a block of iron or ice to make a sculpture. The truth is that most cosmetic, dermatologic, and plastic surgeons are constantly sculpturing while they are performing liposculpture.

The way that I see liposuction  is that the first stage in the procedure is to create a solid block of tissue that can be sculptured. It would be very hard for an artist to sculpture a ball of cotton wool in the same way as he would be sculpturing a block of ice. By infiltrating and filling the fat with the tumescent fluid, the body tissue becomes solid, firm, and expanded. This is now the ideal preparation for a liposculpture procedure. Liposculpture means passing a small cannula that is attached to a suction apparatus in and out, back and forth, up and down, many thousands of times. Each pass is removing another layer of fat. The way to sculpture the body is to pass the cannula at multiple layers, starting deep and moving superficial, changing the angle and the direction of the cannula at every few passes. By repeating this process from multiple directions, the surgeon can evenly deflate and shape the removal of fat to the level which he desires. One of the worst things that could happen with liposculpture is taking too much fat and leaving a depression. Because the fat that is removed is replaced by fibrosis and healing, it would be very hard to re-correct an over liposuction  area. I recently saw a patient who had a liposculpture in Thailand. A surgeon who performs liposculpture must assess how much fat to remove. One of the critical things in making that assessment is to understand each area. Every area of the body can tolerate a certain amount of fat removal. The second thing is, there must be an appreciation of the surrounding skin. The goal of liposuction is to match in the area removed with the surrounding fat. It has to be harmonious; it has to be continuous.
