There are many different types of changes that can occur to the neck and chin liposuction as one ages. Sometimes it is genetic and other times it is related to facial movement and general aging. A common change is called the double chin. People who have a double chin often notice it on photos and some patients will actually wear scarves or high-necked jumpers just to cover their double chin. There is often a very strong genetic element. Patients will say that their mothers had exactly the same appearance. Sometime people want surgery in order to prevent it progressing to look like the way that their mother’s did.
The double chin is composed of 2 components. Usually there is excessive fat as well the second component being loose skin.
WHEN DOES A DOUBLE CHIN START?
It is interesting that I do see patients who are quite young, in their 20’s and 30’s, who can already have the beginnings of a double chin. Sometimes it will just be a little convex surface hanging from the undersurface of the chin. Patients will notice that they can grab the area of fat in that area. With time, the fat tends to accumulate more and eventually become a real double chin.
Is a double chin related to general weight?
Cosmetic and plastic surgeons have noted for years that sometimes localised areas of fat are not related to general weight. Patients might complain that even when they lose a lot of weight, the double chin remains. There are some patients who have in fact just the opposite. With weight loss and dieting, the skin starts to droop and sag. This may become more noticeable on the neck and actually aggravate the double chin.

What are the surgical approaches to a double chin liposuction?
Essentially we have a number of different surgical approaches to the sagging chin. The first is a liposculpture using the Vaser or a superficial liposuction method. We are able to remove essentially nearly all of the fat from the neck. By removing fat, this induces a very tight retraction of the dermis so that there is skin tightening and elevation. Often, this is all that is required. In other patients, if there is excessive skin, then we would consider either a suture lift, where the skin is just sutured to the ear cartilage, or a mini neck and face lift, where the skin is tightened and removed from behind the ear. The latter will give a more permanent and dramatic result.
Are there non-surgical treatments for double chin?
Yes there are so called fat-blasters and skin tightening treatments that can help double chin. One that I have found to be most effective is the Ulthera which is a concentrated ultrasound wave length that goes into the muscle to cause skin tightening. There can be pain with this laser. Also, as it is non-surgical, one can never give an absolute promise that it will be successful.
What are the complications of a surgical neck lift?
All surgical treatments have risks of bruising, pain, and swelling. There is a sensory nerve on the lower part of the neck which is usually out of the territory of the surgical approach, but that is always a potential risk, as well as a small nerve that affects the movement of the mouth. Even with a mini lift mostly, the surgeon stays out of the main nerve territory. However, this remains the primary risk with any surgical face lift.
How many years younger would I look after removing a double chin?
A double chin does give the impression of an aged appearance. When we look at photographs of young children or teenagers, there is always a tight complexion with a sharp angle of the jaw line. When one loses the jaw line sharpness through development of a double chin, the person looks older and often tired and withered. These impressions all change following cosmetic surgery.
